According to page 229 of the 6th edition of APA writing manual, the title page should be formatted as follows. The title page includes five elements: title, running head, author byline, institutional affiliation, and author note. Identify the title page with the page number 1. The remaining pages should be numbered consecutively, using Arabic numerals (except for artwork and figures).

The running head is an abbreviated title that is printed at the top of the pages of a manuscript or published article to identify the article for readers. The running head should be a maximum of 50 characters, counting letters, punctuation, and spaces between words. It should appear flush left in all uppercase letters at the top of the title page and all subsequent pages.

Title page requirements are not limited to the above instructions. Professors or the universities themselves may require additional information not required by APA. All APA documents should have a title page and running header created, if there is not a title page in the original document.
